Questions for Possible GS Candidate What do I ask

My wife and I have had a long and painfull (emotional & physical) journey through the infertility world and now need to look at surrogacy. Ilona is able to produce her own eggs so we are only looking for a GS. I placed an ad on this site a little while ago and have had a couple of responses. I'm hoping to find some sort of "questionaire" (spellcheck) that I might use to get information from the prospective GS. I understand that both Bala & CSO would be able to provide us with a complete profile on any candidates that they have but we think the cost of they're services are beyond on ability right now.

Hi there, I think I might have responded to one of your ads. I have received the application package from Bala and did a face to face interview with CSO. I think I might be able to send you the Bala questionairre if it would help?
